JUVENILE REGISTRATION FORM  This form is for both male and female interested in playing Juvenile in the 2010/2011 season.  If we have enough interest; we will put forth our best efforts to accomodate both males and femalesl.  No payment required at this time_

Please NOTE:

 

• Registration is on a ‘first-come first-serve’ basis.
• All players must be registered in their age division.
• Family Discounts will apply at time of registration.
• Wait Lists for the 2010 Season are in effect. 

* Post-dated Pay Schedule as follows:   June 16, July 15 & Aug. 15

  All Fees must be paid IN FULL by August 31^st, 2010.
