This form must be completed by ALL Managers/Coaches upon completion of EVERY game. It must be submitted via email to Managing Directors/Commissioners within 48 hours. Gamesheet must then be mailed withinthe time directed. Failing to do this will result in a $100 fine to the COACH/TEAM |
